PURPOSE AND SUMMARY
The Life Enrichment Therapy Assistant/Transportation Aide is charged with providing safe and timely transportation services to the clients of the Center. This position will provide transportation to the Center’s clients in and outside the campus as necessary for medical, clinical or recreational appointments, which may be accomplished by hand or with the use of any of the Center’s vehicles. This position is also responsible for providing a program of recreational activities based on the comprehensive assessment and care plan and the preferences of each resident in coordination with the therapeutic plan of care.
Transportation Duties: To provide safe & appropriate transportation of clients to and from medical, clinical and recreational appointments, as well as provide safe & proper client-specific physical assistance while meeting their transportation needs. Provide all the services of a Certified Nursing Assistant while engaged in the transportation of a client whether within or outside of the Center. Provides emergency first aid/crisis intervention as needed. Transports significant information received from schedulers, doctors, nurse, etc. to appropriate persons, including confidential information. Maintains accurate logs for mileage and destination for billing purposes. Keeps vehicle clean. Reports any safety concerns to the Maintenance Department and schedules vehicles for oil changes, etc.
Life Enrichment Duties: Implements recreational programs to support residents in their choice of activities, designed to meet the interest of and support the physical, mental and psychosocial well-being of each resident encouraging both independence and interaction in the community. Documents daily attendance and progress toward defined goals in compliance with State and Federal regulations and completes departmental assignments in a timely manner. Develops and implements therapeutic programming based on the individual needs of the residents, including one on one person-centered care, in accordance with State and Federal regulations and provides recommendations to Life Enrichment Supervisor regarding programming. Assists Life Enrichment Director with completion of time-sensitive MDS documentation to ensure compliance with State and Federal regulations. Assumes responsibility for residents during activities outside the facility; professionally represents the facility when implementing community resource programs. Function as a Certified Nursing Assistant as required by facility need and when down time occurs. Assist residents with activities of daily living; i.e., daily meal, bathing, dressing, etc. and documents pertinent information in the electronic medical record. Distributes resident mail. Attends professional in-services for continuing education and to facilitate departmental knowledge and growth. Attends and contributes in resident care plan conferences as necessary. Participates in continuous quality improvement activities as assigned. Responsible for general appearance of units as needed. May perform other duties or assignments per facility needs.
High School diploma. Preferred Wisconsin certificate as a Nursing Assistant. Current CPR and First Aide Certification. Secure and maintain in good standing a valid Driver’s License in the State of WI. Functional understanding of medical terminology. Good personal communication and organizational skills. Interacts positively with community providers and the general public; performs duties with consideration for residents, peers, other employees and visitors. Organizational skills necessary to assume responsibility for planning and carrying out activity programs. Excellent knowledge of Microsoft Word & Excel. Must have the ability to create posters, spreadsheets and other documents as required. Must have proficient keyboarding skills and computer knowledge. Excellent oral and written communication and interpersonal/leadership skills. Knowledge of ability to initiate, implement and instruct general group activities for special needs populations, i.e. exercise, handicrafts, discussion groups, music groups. Knowledge and understanding of State and Federal nursing home regulations. Understanding of intervention/prevention methods for interacting with those populations with physical, cognitive and/or emotional disabilities. Ability to understand and effectively carry out oral and written instructions. Attention to detail as well as ability to focus on resident needs and daily tasks is essential to this position. Ability to understand and comply with federal, state, and facility safety regulations. Knowledge of all Center Safety Protocols as it related to this position.
