SYNERGY HomeCare Care Team Manager and Scheduling Coordinator

This multi-duty position is responsible for managing the day-to-day operations of a team of caregivers to include scheduling and reporting.


Care Team Manager and Scheduling Coordinator is responsible for the daily duties of managing a team of in-home caregivers and for ensuring that client care schedules are up to date and filled in order to ensure that clients are receiving the highest level of support services and continuity of care.

Primary Responsibilities:

  • Improve employee and client retention rates through active communication and problem-solving efforts
    Become the reliable point of contact for each care provider and client relationship

  • Document skills and attributes of caregivers to ensure strong matching to clients

  • Schedule caregivers with clients and communicate to all parties

  • Train caregivers on how to use the mobile electronic reporting system

  • Innovate and create initiatives to enhance caregiver retention

  • Work with the owners to develop the annual caregiver budget and operational plan

  • Manage and support all care team members

  • Responsible for in-person Client/Caregiver introductions

  • Responsible to cover any assignment not filled by team members

  • Participate in management on-call, after-hours rotation

  • Observe all state and federal guidelines and licensing requirements for compliance

  • Develop and maintain positive relationships within the healthcare and referral community

  • Client and caregiver documentation in scheduling software

  • Responds to questions regarding client and caregiver schedules

  • Confirm and update caregiver availability including vacation, sick time and holidays

  • Follow up with new caregiver hires to explain the scheduling process

  • Update the office team on scheduling issues and HR recruitment needs

  • Maintain computer schedules and ensure timely data entry for clients and caregivers

  • Maintain confidentiality of all information pertaining to employees and clients

  • Perform other functions as deemed appropriate by the management team

Required Skills and Experience:

  • Valid Wisconsin Driver's License with clean driving record.

  • Reliable, low maintenance vehicle to commute to/from client homes and office

  • Experience in home care or home health agency required

  • 3+ years proven experience in management/leadership position required

  • High School graduate or equivalent required, Associate degree preferred

  • Scheduling experience required

  • National Background check required

  • CNA Required

  • Strong phone skills required

  • Ability to multitask and work well under time pressure

  • Positive, friendly attitude and ability to work well in a close team environment

  • Conducts Weekly Reporting

  • Verifies timecards/telephony for correct hours, authorized signature and completeness on a daily basis
