Job Description
Rocky Knoll Health Care Center is seeking a Full-time and Part- time CNA’s.
 
This position has a starting salary of $16.00/hour.
 
Full and part time positions on 2nd & 3rd shift, and weekend only positions. Hours vary based on need. Benefit eligible at 18+ hours per week. **** $1,000 SIGN ON BONUS for full time positions hired in March of 2020 for PM shift and night shifts, $250 for day shift (paid at 6 months, and 1 year). On-site Daycare opening summer of 2020.
 
PURPOSE AND SUMMARY
Provides comprehensive nursing care to residents by performing routine and simple tasks under the direction of licensed staff. Supports the principles of the nursing process, standards of nursing practice, current state regulations as per HFS 132 and federal regulations, and / or the program objectives for the diversified population of the facility.
 
ESSENTIAL DUTIES
Promotes a concept of person-centered care for the residents of the facility. Assists residents with all grooming habits including bathing, dressing, brushing hair, toileting etc. Assist residents with eating and drinking at mealtimes. Monitor and record resident daily activities and reports any drastic changes to the unit nurse. Completes documentation/charting as needed or assigned. Assist with any daily exercises or mobilization efforts resident has been prescribed. Ensure residents are comfortable in bed; adjust bed settings and/or reposition/turn residents as needed. Help transport residents, including lifting them out of bed to use a walker or wheelchair. Talk with residents and their families, providing them with company and listening to their questions and concerns. Maintain supplies, restock when needed and notify supervisor when running low. Answer resident call bells/lights/alarms and take necessary action according to facility protocol. Reasonable anticipated risk of occupational exposure to blood and/or other potentially infectious materials. Attends and contributes in resident care plan conferences and Quality Assurance meetings as necessary. Performs approaches identified in care plans. Responsible for general appearance of units as needed and performs emergency cleaning as assigned. Cleans wheelchairs and other equipment as assigned. May attend in-service education programs offsite as well as attends in-service provided by the facility in order to update their skills. Assists in providing orientation to nursing staff. Responsible for carrying walkie-talkie during scheduled shift and having on at all times. Follows all facility Infection Control guidelines. Takes weights, heights, etc. and records eating habits and bodily functions. Change linens and provide new bedding, blankets and pillows. Check work email on a consistent basis. Follows state and federal mandated standards and precautions. May perform other varied duties/assignments per facility needs.
 
QUALIFICATIONS
High school graduate or equivalent experience preferred. Current Wisconsin certificate as a Nursing Assistant including annual 12 hour educational requirement. Ability to handle crisis intervention situations as directed by individualized care plan, facility protocol, resident rights, etc. Basic level of computer proficiency required. Functional understanding of medical terminology. Understands and effectively carries out oral and written instructions. Understands and promotes the concept of comprehensive individualized nursing care for a diversified population.
