PURPOSE AND SUMMARY
This position is a part of the clinical treatment team of the Mental Health & Substance Abuse Treatment Center. This is an advanced position within the team, capable of working independently with challenging cases and/or in difficult circumstances as well as supporting the education and supervision of agency staff with regard to evaluation and treatment of addiction. AODA Counselors provide a full continuum of outpatient services to individuals seeking addiction treatment. This includes providing evaluation and treatment services to individuals and their families, participating in outreach and prevention programs, providing AODA consultation, information and referral services and supporting program development. As the Mental Health & Substance Abuse Treatment Center is currently the location of central Intake for the Division of Community Programs (DCP), AODA Counselors also take inquiry calls and meet with individuals who walk-in to the Treatment Center to apply for services. If these individuals are in crisis, AODA counselors will provide crisis intervention and/or create a plan in collaboration with one of our community crisis service partners to ensure safety and support stabilization.
Completes comprehensive initial assessments that reflect the client’s need for outpatient addiction treatment services to include presenting problems, substance use history, psychosocial history, diagnostic impressions, areas of functional impairment, and recommendations for treatment. Recommendations for treatment include application of the approved placement criteria and review of the diagnosis and level of care recommended by the medical director (staff psychiatrist) or a licensed clinical psychologist. If an AODA counselor identifies symptoms of a mental health disorder and trauma in the assessment process, the service shall refer the individual for a mental health assessment conducted by a mental health professional. Provides results of the assessment to the client (and/or parent/legal representative) as well as treatment alternatives, client’s rights and responsibilities, the anticipated fees for treatment, and the clinic’s grievance procedure. Develops a treatment plan in collaboration with the client based on the initial comprehensive assessment and describe the patient’s individual or distinct problems. The treatment plan shall specify short- and long−term individualized treatment goals that are expressed in behavioral and measurable terms, and are explained as necessary in a manner that is understandable to the patient. The treatment plan should include realistic goals, expected outcomes, the treatment, rehabilitation, and other therapeutic interventions needed to reach the client’s treatment goals, and describe the criteria for discharge from services. Ability to abide by the state rules and regulations regarding client confidentiality and client rights as they relate to AODA services. Conducts treatment plan reviews (staffing reports per DHS 75.03) with the client to evaluate progress toward goals, the need for additional assessment or changes in the plan, and verification of current medications. Clinical treatment plan reviews must be scheduled every 90 days for clients who attend treatment one day per week or less and every 30 days if clients are seen more than once per week. Participates in the on-call coverage that supports clinic’s response to telephone calls and individuals walking in to obtain information about mental illness, addiction, and treatment and/or to apply for services. A minimum of 4.5 hours per week is expected by each staff member to cover all hours of clinic operation Monday through Friday. Provides crisis intervention to individuals who present to the clinic in crisis which may include development of a crisis plan and/or collaboration with our crisis system service partners in the community. Provides individual, couples/relationship, family and group psychotherapy to clients based on their treatment/recovery plan using evidence-based, best practice models of treatment and therapeutic intervention. Group therapy programs will be developed based on need for a specific service based on referrals to the treatment center. Evaluates and provides individual counseling, group therapy, marital and family therapy to address the impact of substance abuse/dependence on the client and/or his/her family. Travels to the Sheboygan County Detention Center for the purpose of completing assessments for the Drug & Alcohol Treatment Court Program, the Opioid Detoxification and Treatment Program, and to facilitate groups as needed. Provides information about substance abuse/dependence and about AODA services to prospective clients, family members, or other interested individuals. Provides consultation to agency staff and community partners regarding the dynamics of and assessment and interventions used with individuals struggling with substance use disorders.
Performs basic case management functions for assigned clients including:
- collaborating with other providers working with client from other agencies regarding treatment and aftercare planning
- assisting clients in residential care to return to the community
- providing advocacy to facilitate a client’s adjustment in the community by providing education to employers, family members, landlords, physicians, community agencies, and aftercare resources
- referring client to case management services within DCP if case management needs exceed that which can be provided within counselor role.
Maintains client’s electronic record in accordance with confidentiality rules and regulatory requirements and standards. Timely completion of documentation is required to include the initial (and subsequent) assessments, progress notes, staffing/supervision meetings, collegial clinical collaboration, treatment plans and reviews, case management, education, referrals, collateral contacts, phone calls/texts/emails, and a closing/discharge summary. If credentialed as an Intermediate or Independent Clinical Supervisor, provides supervision to other AODA staff as directed by supervisor. Provision of such supervision includes monitoring of and responsibility for all activities performed by the supervisee and should follow guidelines for the provision of clinical supervision including associated documentation as outlined by the Wisconsin Department of Safety and Professional Services. Provides outreach and/or crisis services in situations where an AODA client may be experiencing difficulty to facilitate his/her entry into an appropriate level of services. Provides information and referral and crisis intervention services to callers and walk-ins to the Mental Health & Substance Abuse Treatment Center during assigned on-call time. Some crisis situations may involve physically dangerous circumstances. Provides leadership and input into the planning of new programs and in-service training. May be assigned duties of Intoxicated Driver Program Coordinator. Performs other duties within the scope of professional responsibilities as assigned by supervisor.
Certification through the Department of Safety and Professional Services as a Clinical Substance Abuse Counselor, Substance Abuse Counselor, or Substance Abuse Counselor In-Training required, with preference given to individuals with Clinical Substance Abuse Counselor or Substance Abuse Counselors. Bachelor’s or Master’s degree in a relevant Human Services field from an accredited school required. Master’s degree and licensure through the Department of Safety and Professional Services as a social worker, marriage and family therapist or professional counselor is preferred. Extensive experience working with individuals diagnosed with substance abuse or dependence with experience in an agency setting providing evaluation and treatment services preferred. Knowledge and understanding of the dynamics of and assessment and interventions used with individuals struggling with substance abuse or dependence. Some knowledge and understanding of mental health concepts preferred. The ability to relate and communicate effectively with clients, staff, other agencies, and the general public; demonstrated reliability, resourcefulness, and good professional judgment. The ability to communicate well in written form in order to complete documentation of services provided letters, and reports using appropriate grammar, punctuation and organization. The ability to abide by the state rules and regulations regarding client confidentiality and client rights as they relate to AODA services. Unlimited access to a car, a valid driver’s license, and ability to drive or arrange travel in and out-of-county, as needed, and requested by Supervisor or his/her designee. Ability to establish positive connections and develop relationships with clients, their natural supports, and other professionals working within our continuum of services. Ability to be sensitive to and respectful of the life experiences, individual differences, belief systems and practices, and the motivation for change of each client. The capacity to meet each client where they are and instill hope for recovery. Ability to maintain a calm demeanor and make good clinical decisions in crisis situations while adhering to clinical, ethical, and regulatory standards. Ability to and comfort with seeking consultation from others when needed. Ability to communicate with persons from diverse cultural and socio-economic background. Excellent communication skills. Aptitude for critical thinking. Ability to triage patient needs and prioritize tasks, activities, and service provision according to clinical acuity. Strong knowledge base in recovery principles, person-centered planning, and trauma informed care. Capacity to uphold the professional code of ethics and HIPAA standards for the protection of personal health information in all service provision. Ability to provide excellent customer service.
Regular contact with clients and their family/natural supports, Division of Community Programs (DCP) clinicians and case managers, representatives from our crisis services continuum, other SCHHS staff, community service partners, and members of the public. May occasionally deal with aggressive or violent individuals.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS
Work is performed primarily in a standard office setting, but with some work in a variety of settings in the community. Some travel is required. Ability to frequently lift up to 20 pounds and a combination of sitting and standing postures may be required. Must be able to operate standard office equipment including telephone, computer, facsimile, calculator, printer, and photocopier. Risk of blood-borne pathogen exposure is considered moderate.
