We have Full time and part time openings on multiple shifts. 

 The LPN and RN position provides direct nursing care to our residents, and supervises the day-to-day nursing activities performed by nursing assistants ensuring that the highest degree of quality care is maintained in accordance with the resident’s care plan.

Essential Functions/Responsibilities:

• Direct the day-today functions of the nursing assistant and make sure cares are being provided according to the residents care plan and wishes
• Receive physician's orders and execute as appropriate
• Chart nurses' notes in an informative and descriptive manner that reflects the care provided to each resident in EMR
• Ensure appropriate documentation and communication of changes in a resident's needs and/or condition
• Prepare and administer medications as ordered by the physician
• Ensure that an adequate supply of supplies and equipment is on hand to meet the nursing needs of the residents
• Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician
• Review the resident's EMR for specific treatments, medication orders, diets, etc.
• Notify the resident's physician and P.O.A./Guardian of changes in condition
• Administer professional services such as; catheterization, tube feedings, wound care, etc.
• Follow infection control and universal precautions policies and procedures to ensure that a sanitary environment is maintained at all times
• Attend and participate in required training, conducted at time of hire and/or annually, that will include training designed to assure resident safety: i.e. fire safety, missing resident, evacuations, disaster response, etc.

Requirements

• Possess a current, active license to practice as an LPN or RN in the State of WI
• Understands and supports Pine Haven’s Mission, Vision and Values and Resident-Centered Engagement Principles
• Regular attendance, including timely arrival at work, to support positive working relationships with fellow team members and to support quality resident care
• Ability to work in a fast paced rapidly changing environment
• Attention-to-detail and organizational skills required
• Strong teamwork and communication skills are critical
• Ability to lift up to 40 pounds
