At Embrace Care Management it is our Mission and commitment to providing compassionate, dignified, and ethical care, ensuring peace of mind for seniors and their families. Aging can be overwhelming but you or your loved ones and your clients do not need to go through this alone. Embrace is here to help. Our dedicated Social Workers and Nurses are prepared to answer the difficult questions related to aging. Questions such as "should Mom stay in her own home"? or "how do we handle Dad's memory loss?" We are able to act as an advocate, to assist when a family lives out of town or when the family is unsure how to move forward.

In addition to our Care Management services, we offer Managed Companion Care. This flexible service has availability 24/7/365. Our companions can help with you or your loved one's/ or your client's day-to-day needs such as dressing, bathing, transportation, cleaning, meal preparation, and perhaps most importantly companionship.
