The Sheboygan Family Resource Center has released a short, confidential survey to gather feedback from families participating in its Parents as Teachers programs. The survey is part of the Center’s ongoing effort to better understand the experiences, values, and needs of the families it serves, and to ensure its communication, resources, and support services remain welcoming, inclusive, and culturally responsive.

We are asking families to participate. The survey takes only a few minutes to complete and is available in both English and Spanish. The link can be found here. 

At the end of the survey, participants may volunteer to take part in a brief follow-up interview. Those who complete an interview will receive a $50 gift card to either Amazon or Walmart—their choice. Contact information for interviews will be kept confidential and stored separately from survey responses.
